Both teams knew where the second runner was. Each had to calculate the risk of getting him in a pickle and out while potentially letting the lead runner score. The lower-probability passed ball created a bonus opportunity for the double score.
They must have been playing the absolute worst team in their division and planned it. Too bad we didn’t get the final score of that game. Had to be a blowout!

Coaches fault
The pitcher was aware he could have thrown the ball to 2nd base for a out.
That was ALL on the catcher who controls the entire infield. The catcher should've checked the runner on second and alerted the pitcher asap.
